The following form should be used to request an ad-hoc meeting schedule on the Board of Regents multi-point video conferencing equipment. This form should not be used to modify an existing schedule request, modification requests should be made via email to the Board of Regents Video Staff at firstname.lastname@example.org.

Meeting coordinators should collect all information for this meeting (including all room assignments) before filling out and submitting this form.
This form has space for 15 room assignments. If multiple participants will be in the same room, the room assignment should only be included once. If more room assignments are needed, please contact the Board of Regents Video Staff directly at email@example.com.

Room assignments should be entered as: institution, ip address, physical location (if available), coordinator contact information. Examples:
